How to get pregnant if your husband has low sperm count. Spermogram: is it possible to get pregnant. You have serious problems with potency

A spermogram is important when planning a pregnancy. How to get pregnant with a bad spermogram? What should be the spermogram to conceive a child?

The process of conceiving a child for many couples is increasingly becoming problematic, and the appearance of a long-awaited baby remains a dream. Turning to the doctor and passing a whole bunch of tests, in half of the cases it turns out that the woman has no health problems, and the reason for unsuccessful attempts to have children is that the spermogram is not at all suitable for conceiving a partner. Pregnancy may not occur with a bad spermogram. That is why a family planning spermogram is mandatory if young people are interested.

Is it possible to get pregnant with bad spermogram?

It all depends on the quality of the spermogram for conception, which determines the fertility of a man. Back in 2010, the World Health Organization presented spermogram data, in which the chance of getting pregnant naturally is high. However, today infertility in men, unfortunately, is not so rare, so you should definitely consult a doctor and find out how to improve the quality of spermogram for conception.

Is it possible to get pregnant if the spermogram is bad? Yes, it is possible if the indicators have only minor deviations. As a rule, the Ph level of the medium is violated or the viscosity of the ejaculate (semen, seminal fluid) is exceeded. This is a consequence of any inflammatory processes in the body or minor diseases. Therefore, for a successful pregnancy, you just need to be cured by drinking a course of drugs prescribed by a specialist, and then the spermogram for conceiving a child will be much better. That is why turning to the family planning center, a spermogram is simply necessary if there are problems.

How to improve the quality of sperm for conception

You can improve the quality of the spermogram with folk remedies for conception, but this takes time. A huge role is played by spermogram indicators for conception, because conception with a poor spermogram in some cases is hampered by a small volume of ejaculate or a low concentration of spermatozoa (less than 12 million in 1 ml). This can be caused by a hormonal failure or injury, which, of course, is successfully treated and after that you can be tested again, because normal sperm counts for conception guarantee a positive result. In addition to all this, you can also find information on how to improve spermogram for conception, read reviews and decide for yourself which method is best suited.

Much more serious if elevated leukocytes or low sperm motility are detected. If the male cells move slowly, it is extremely difficult to get pregnant naturally, and most experts advise resorting to IVF in combination with the ICSI system. Sperm immobility is associated with poor genetics or harmful living and working conditions, ecology and other reasons. In turn, a high content of leukocytes in semen indicates the presence of diseases of the genitourinary system that require immediate treatment.

Thus, the answer to the question what is the probability if a bad spermogram does not become pregnant depends on the reasons for the decrease in indicators below the norm. Identification and struggle with them become the basis of appropriate therapy. Be sure to contact the family planning center to pass the spermogram again.

What can be done to improve sperm quality?

Many couples, after receiving the results of the spermogram, begin a course of treatment, since it is the normal spermogram that is best for conception. At the same time, ways to solve a problematic issue necessarily include a number of useful recommendations, the observance of which will increase the chances of conception:

  • To improve the quality of sperm for conception, you just need to give up alcohol and smoking;
  • Proper nutrition will help increase the quality of spermogram for conception. The diet should be discussed with a nutritionist who will recommend what food to focus on to improve men's health;
  • Priority on a healthy diet, especially on foods high in zinc (lean beef and lamb, peanuts, sesame, veal liver, pumpkin seeds, pollock);
  • Reduce heavy physical exertion, but do not neglect an active lifestyle, many are looking for information on how to improve spermogram for conception with folk remedies and this method suits them too;
  • Do not fall into depression and discouragement, because the emotional state plays a big role in achieving a positive result. It is calmness that leads to the fact that after a while the spermogram for conception turns out to be normal, and the woman should in every possible way support her man in the fight against this problem and be sure to ask the doctor what the spermogram should be for conception and go towards this goal.

A spermogram is the main analysis that is prescribed to a man if he is suspected of having a diagnosis of infertility. This method allows you to identify male infertility. Therefore, the answer to the question “is it possible to get pregnant with a bad spermogram” will be obvious.

A spermogram for infertility is also prescribed during the period of preparation for IVF or when planning a pregnancy. Based on its results, the doctor can give an exhaustive answer why it does not occur pregnancy subject to regular unprotected sex.

Why can a spermogram show bad results?

If the spermogram is bad - do not get pregnant. This is a statement of not quite knowledgeable people. In fact, there are indicators that affect the fertility of a man, and indicators that do not really matter.

But, first, let's consider the question, why the results of a sperm study can show a bad clinical picture?

  • Alcohol abuse.
  • Systematic smoking up to one or more packs of cigarettes per day;
  • Unfavorable conditions at work, under which exposure to chemicals.
  • Hypothermia of the organs of the genitourinary system, which entailed the development of inflammatory processes.
  • Regular overheating of the genitals, especially the testicles, which produce sperm. This may be due to work or wearing clothes that are too warm and tight.
  • Frequent trips to the bath, sauna, solarium, as well as taking hot baths.
  • Sedentary lifestyle.
  • Excess body weight.
  • Wrong nutrition.
  • Taking strong drugs and so on.

Often, spermogram indicators worsen against the background of the presence of inflammatory processes in the pelvic organs. For this reason, male infertility is not diagnosed. Spermogram will be better after undergoing effective treatment prescribed by a doctor.

Is spermogram effective in infertility?

When a man turns to a specialist with the problem of the inability to conceive a child, the first thing he does is appoint a detailed analysis of the sperm. After preparing for the procedure in accordance with all the rules, the man takes the test and waits for the results.

What results can indicate absolute infertility:

  • Absence of spermatozoa in the semen sample;
  • A significant decrease in the percentage of the presence of live spermatozoa;
  • Total decrease in actively motile spermatozoa;
  • Gene mutations in sperm;
  • Agglutination confirmed by several tests, etc.

If the husband’s spermogram turned out to be so bad, the doctor will tell you how to get pregnant. You may have to use donor genetic material.

What indicators can be corrected after treatment?

If, according to the results of the analysis, the presence of an inflammatory process is revealed, then only the correct effective treatment will help improve sperm parameters. For such purposes, antibiotics are prescribed, as well as drugs that can affect the quality of sperm. After treating the underlying problem, the patient is prescribed a second spermogram. Depending on the chosen treatment tactics, the period between procedures can be 2 weeks-3 months.

It is possible to conceive with a bad spermogram and you can get pregnant if you improve your lifestyle and get rid of bad habits before the second procedure. The last factor affects such indicators of spermogram:

  • The amount of sperm;
  • Her quality;
  • sperm motility;
  • their survival;
  • The number of sperm in 1 ml of biological material.

In other words, a person who drinks or smokes frequently has a significantly reduced chance of conceiving a child.

What is the probability of pregnancy with poor spermogram?

Many couples are interested in if the spermogram is bad, is it possible to get pregnant? It should be noted right away that not all indicators of the study pay special attention to male fertility. For example, if minor morphological changes are found, then with the required amount of sperm and its low viscosity, conception can occur.

If the changes are significant, and there are suspicions of a genetic pathology, then the pregnancy may end in a miscarriage, despite the fact that it does occur.

When planning pregnancy and its long absence, one should not rely on chance. If, when all conditions are met, conception does not occur, then you already need to be wary. With the timely elimination of the problem, the chances of a successful conception will increase significantly. To understand whether there can be a pregnancy with a bad spermogram, a doctor will help, who will prescribe additional tests and re-examination of the sperm.

How to get pregnant if the husband has a spermogram with deviations from normal values?

As mentioned above, there are important and unimportant indicators of male fertility. If the results of the sperm study show deviations from the norm, while the alleged diagnosis is not confirmed by other tests, then there is every chance of becoming pregnant even with a poor spermogram.

If the long-awaited pregnancy does not occur, you can try to change the diet. Here are some recommendations regarding menu planning for men:

  • You need to include fresh fruits and vegetables in your daily diet;
  • Nuts will help improve sperm counts, even if they are within the normal range according to the results of the study;
  • Preference should be given to lean meats;
  • The liver contributes to the normalization of blood circulation;
  • You need to add more greens to dishes, especially parsley;
  • Whole grain products, as well as legumes, which include buckwheat, wheat, rice, lentils, and oats, will go in favor.

Before getting pregnant with a bad spermogram, it is better to consult a doctor. Perhaps he will prescribe a second sperm test in another clinic, where the collection of ejaculate is provided under strict laboratory conditions. To undergo a reanalysis, you need to prepare in the same way as for the study conducted earlier.

Expert opinion With symptoms such as traces of pus in the seed, you need to temporarily completely abandon plans to conceive a baby. It is advisable to at least use tight condoms, and it is even better to ejaculate outside the vagina at the same time.

Why take the risk if you can wait a bit until the quality indicators of spermatozoa return to normal.

Sperm is a physiological fluid that is produced in each man by certain genital organs. The presented biological substance is seminal fluid, in which the main components are spermatozoa, as well as a number of other components.

All components of seminal fluid are organic and produced by the testicles of the male body. When the presence of male gametes in the biological fluid is significantly reduced, then the sperm turns into a viscous, completely transparent liquid. A man begins to produce sperm during puberty, this age starts from 12 years old and ends in old age.

Men often ask the question, under normal conditions of life, what should be the seminal fluid, what color is normal sperm? So, for all men everything is different, it has different color shades, it all depends on the diet and sphere of life, sexual activity. The sexual secret of a man is most often a whitish-gray cloudy viscous viscous mucus, heterogeneous and sticky in consistency, with a certain characteristic odor. If we compare this smell with something, then it can be compared with the smell of raw chestnut, this smell is given to it by the prostate gland, since the secret of sperine is produced in it, having a slight smell of chlorine. Sperm is not completely liquid, it is represented by dense mucus, if you look at it after a period of time, you can see more liquid structures, since it liquefies under the action of enzymes. But there are times when a man notes changes in the color of the seminal fluid, but this does not mean at all that this indicates a disease. Seminal fluid has numerous shades: pale yellow, straw. The color of semen normally depends on the content of spermatozoa, namely, sperm is ejaculate, in books it is described as a whitish liquid. If the content of the ejaculate is reduced, then the sperm acquires a brighter yellow color, if the sperm content is very low, then the color of the seminal fluid will be greenish, but if the concentration of the ejaculate is sharply increased, then the sperm becomes not white, but gray, I would say, darker color. But there are other cases when semen becomes pink, this condition is manifested by the sweating of erythrocytes (red blood cells) into the seminal fluid. This condition is called hemospermia. If the color of the semen changes to redder, then the urine also acquires a different color, blood can enter the semen anywhere in the body, especially if inflammation is localized in it.

This condition is unfavorable for a man and requires immediate medical attention. Since the next stage will be brown sperm, this is already the predominance of a chronic process or a long-term illness, such a shade may indicate cancer.

A greenish color in the seminal fluid appears when an infection enters the genitals. Infections of the genital or urinary system can be complicated by purulent fusion, then an unpleasant odor will join the shade of the seminal fluid.
By the shade of sperm, you can also analyze your own health and reconsider your lifestyle, diet, change the regularity of your sexual life, but of course, a spermogram will certainly be a diagnostic confirmation of the pathology. Depending on the results of the examination, the specialist will choose the examination and further treatment tactics.

There are examples when sperm changes its color, this is provoked by both infectious diseases and lifestyle. The color of the biological fluid can change from taking medications, especially if you pay attention to the shade of sperm while taking antibacterial drugs, the color becomes yellow. As mentioned above, the shade of the ejaculate also depends on the diet, especially if you use dyes or coloring ingredients, beets, carrots and others, a change in the color of urine can often be observed. The shade of sperm is affected not only by diet and medication, but also by the frequency of sexual intercourse. If a man had a long abstinence, then the shade of the sperm will be more saturated and dark, with the possession of opalescence. But if there are a large number of sexual contacts, then the shade of the sperm will be more transparent and the structure itself will be more liquid, without signs of opalescence.

Sperm in its composition has several main components - ejaculate (or it is called sex plasma), spermatozoa and germ cells of the testicles (shaped elements of the genital organs).

The composition of the biological fluid also includes proteins in large quantities, citrates, enzymes, flavins, amino acids, phosphorus, potassium. Fructose is part of the semen only to provide nutrition to the spermatozoa. Also, the composition of the sperm is enriched with the secrets of the prostate gland: phosphatase acid, calcium, sodium, proteolytic enzymes. But zinc, as a separate element, plays an important role in the life of sperm - it increases the ability of the sperm to fertilize.

Since ancient times, it was believed that if a couple does not have children, then the woman is to blame. With the help of modern diagnostic methods, it was possible to establish that in about half of the cases, infertility in a couple occurs due to the fault of a man. In this case, using a spermogram, doctors diagnose a deterioration in the quality of sperm.

For the onset of pregnancy, it is not enough only the health of a woman. In order for spermatozoa to be able to enter the uterus, they must be mobile, have the correct shape. The ejaculate also plays an important role. Pregnancy with poor spermogram rarely occurs, so a man must definitely undergo treatment. Consider what a negative analysis looks like, and what you need to do to get pregnant with a bad spermogram.

Spermogram is a modern method for studying ejaculate, which allows you to quickly and accurately assess the composition and quality of sperm. With the help of a spermogram, a laboratory assistant can count the number of sperm, evaluate their external structure, mobility, and also check whether there are leukocytes, blood cells in the ejaculate, and evaluate viscosity.

Any deviation from the norm can cause problems with natural conception. Consider what results a patient can get after passing a spermogram:

  • normospermia - an excellent result, high-quality sperm, the man is ready to conceive;
  • normozoospermia - there are small deviations that do not affect the process of conception;
  • leukocytospermia - there are too many white blood cells in the ejaculate, which indicates an infection;
  • oligospermia - very little ejaculate;
  • oligozoospermia - there are few spermatozoa in the ejaculate;
  • necrozoospermia - there are no live gametes in the semen;
  • azoospermia - there are no spermatozoa in the semen at all;
  • asthenozoospermia - sperm are too slow;
  • akinozoospermia - all gametes are immobile;
  • teratozoospermia - a lot of sperm with impaired morphology (structure).

It often happens that a man, according to the results of a spermogram, has several deviations at once. Whether it is possible to get pregnant with oligoteratozoospermia depends on the cause of the disorder, and on how the man will be treated. With such a violation, there are few gametes, and most of them are not capable of conception, since the morphology is disturbed.

It is worth noting that the result may be poor if the patient did not take the tests correctly. If a man has violations, then it is necessary to retake the spermogram, as well as undergo a comprehensive examination.

There are a few things to keep in mind when submitting an analysis. First of all, it is better to donate sperm in a clinic, so that it goes to the laboratory immediately. Before giving up, you can not have sex for 3 days, so that spermatozoa accumulate in the testicles. It is also forbidden to bathe in a bath, lie in a hot bath, be stressed, drink alcohol, smoke. All these factors inhibit spermatogenesis and provoke a deterioration in the result.

Causes

Is it possible to get pregnant if the husband has a bad spermogram, depends on the reason for this result. If there are very few gametes in the sperm, then this may be due to hormonal disorders, in particular, with reduced testosterone production. This occurs with endocrine pathology, with varicocele. If it is possible to normalize the hormonal background, then most likely spermatogenesis will be restored.

If there are no spermatozoa at all in the ejaculate, this may be due to the adhesive process in the vas deferens. Also, the cause may be retrograde ejaculation and severe testicular pathology, in which spermatozoa are not produced at all. If sperm do not enter the ejaculate due to adhesions, then it will be possible to get pregnant. If there is a problem with the testicles, and sperm are not produced, then it will not work to get pregnant.

Those who have become pregnant with a bad spermogram know that if the spermatozoa are immobile or not alive, then this is a very serious violation. This can occur after chemotherapy, against the background of inflammatory processes, endocrine disorders. In this case, you may not be able to get pregnant, or you will have to use ART methods.

If, according to the results of the spermogram, too many gametes with poor morphology were found, then it is most likely that it will not be possible to conceive naturally. This condition occurs when the body is exposed to various gases, chemicals, with beriberi and problems with the organs of the reproductive system.

It is possible to accurately identify the cause of the pathology only with a comprehensive examination. It is impossible to make a diagnosis based on a spermogram alone, so a man will be sent to donate blood, urine, undergo an ultrasound of the small pelvis and other necessary studies.

Treatment

How to get pregnant with a bad spermogram, a reproductive specialist should tell you, since in this case everything is individual. As a rule, patients are prescribed a complex treatment that helps to improve the quality of the ejaculate, as well as eliminate the main cause of the disorder.

To eliminate the cause of poor spermogram, anti-inflammatory therapy, antibiotics are prescribed, and surgery, chemotherapy, and prostate massage may be indicated. For example, antibiotics will be prescribed for STDs, and surgery for varicocele, and anti-inflammatory drugs for prostatitis.

To improve the spermogram, the patient is prescribed to lead a healthy lifestyle and take vitamins. A healthy lifestyle means proper nutrition, exercise, and the rejection of bad habits. proper conduct of sexual life, maintaining a psychologically stable state.

A man should not eat fatty, spicy, smoked, fried, sweet. Foods with preservatives and other harmful additives, sweet soda, coffee, and alcoholic beverages should be avoided. It is strictly forbidden to smoke and use drugs.

Sex should be done every 3 days, except for the period of exacerbation of various diseases and menstruation in a woman. It is necessary to avoid sexual contact with a non-permanent partner, or be sure to protect yourself with a condom. It is advisable to avoid anal and oral contact, due to the increased risk of developing infectious diseases.

It is very important to exercise, but not too active. Every day you need to do exercises with an emphasis on the muscles of the abdomen, pelvis, buttocks in order to improve blood circulation in the genitals.

It is necessary to avoid stressful situations, try to lead a measured life, enjoy every day. It is also necessary to strengthen the immune system, try to avoid infection with viral and bacterial infections.

To improve sperm quality, doctors may recommend drinking the following dietary supplements:

  • Androdoz;
  • L-carnitine;
  • Verona;
  • spermactive;
  • Folic acid;
  • Speman;
  • Profitil;
  • Spermaktin and others.

Currently, there are a large number of drugs for men aimed at improving the quality of spermogram. If the husband has a bad spermogram, it is better not to self-medicate and not to buy drugs on your own. Because vitamins will not be able to eliminate the true cause of the violation, the patient will only lose time, and the pathology will progress.

ART

Is it possible to get pregnant with a bad spermogram, it is impossible to say exactly. It all depends on whether the violations in the body of a man are reversible, and whether he follows all the doctor's recommendations during the treatment period. If a patient is diagnosed with chronic pathologies, then it is possible to improve the quality of sperm for a while, with minor violations after the cause has been eliminated, even a natural pregnancy is possible.

If sperm quality has not been improved, then assisted reproductive technologies can help a couple conceive a child. With minor deviations, insemination is carried out, that is, spermatozoa are injected directly into the uterus, and then they themselves get to the fallopian tubes and fertilize the egg.

If the sperm are inactive, then insemination will not give an effect, since the gametes will not physically be able to reach the target. In this case, IVF is recommended. That is, the doctor takes the egg from the ovary, and fertilizes it with sperm in a test tube.

In the worst case, when sperm cannot break through the protective layer of the egg even in a test tube, IVF with ICSI can help. Then the doctor will inject the male gamete directly into the egg, after selecting the most viable one. There are additional methods that allow you to evaluate the quality of germ cells and the resulting embryos, thus excluding abnormal and low-quality ones.

The embryo is transferred to the uterus 3-5 days after fertilization, when it becomes multicellular and is ready to be attached to the uterine wall. IVF helps to get pregnant even with severe deviations in sperm quality. The only condition is the presence of at least a few viable spermatozoa in the ejaculate or in the testicles.

If there are no sperm in the ejaculate, then they can be taken by puncture directly from the testicle. If there are no live sperm at all, even in the testicles, then the patient will never be able to conceive a child. In this case, IVF with donor sperm may be recommended.

Thus, pregnancy with poor spermogram is quite possible. But the couple needs to be responsible for the planning process, complete the full course of treatment, lead a healthy and active lifestyle, and also be tuned in to a positive result.

If a woman has no health problems, hormones are perfect, and menstruation goes by the clock, but conception does not occur, then in order to become pregnant, a man needs to pass a spermogram. Then you need to decipher it together with the doctor and start looking for a solution to the problem.

If slight deviations are visible in the spermogram, for example, there are changes in the pH level or the ejaculant is too viscous, then it is necessary to exclude the presence of inflammatory diseases and processes in the future father. When such problems are identified, you just need to be cured. Although in the vast majority of cases, a healthy woman will be able to successfully conceive with similar indicators of a man's spermogram.

If sperm counts are poor

If your husband has too little ejaculate, then the spermogram is really bad. The reason for this can be both inflammatory diseases and congenital pathologies. However, before you panic, you need to duplicate the tests. And in this case, the only answer to the question of whether it is possible to get pregnant if the husband has a bad spermogram will be IVF.

Sperm concentration

Many women became pregnant with a poor spermogram of the husband, when it was caused by a small concentration of spermatozoa (about 20,000,000 per 1 ml of semen).
Both an injury and a hormonal failure, which can be successfully treated, can cause poor concentration.

Low sperm motility

Another indicator that speaks about the quality of sperm is sperm motility. If they move little, then the chances of conceiving a baby decrease, and vice versa. Sperm immobility is affected by bad habits, bad genetics and harmful working conditions. In this case, the couple is advised to use
IVF in combination with the ICSI system.

The option of in vitro fertilization, together with the sperm selection system, is suitable for those couples in which the spermatozoa in men are practically immobile. Before IVF, it is necessary to conduct a full examination cycle and stimulate both a man and a woman.

Elevated white blood cells

How to get pregnant if the husband has a bad spermogram with high white blood cells? In this case, it is necessary to identify an infection or disease of the genitourinary system (namely, white blood cells in semen speak about this) and cure it.

Causes of poor spermogram

If we summarize all the information mentioned above, then the causes of poor spermogram are genetic abnormalities and past injuries, inflammatory and infectious diseases, as well as bad habits and difficult working and living conditions. However, regardless of the cause of poor spermogram, do not despair. It is necessary to minimize all adverse factors, cure diseases and duplicate tests. And in some cases, it’s enough just to go on vacation and just enjoy life and intimacy.

Spermogram is the first and simplest analysis, which is recommended to be taken if a couple has not been able to conceive a child for a long time. Examination of a woman is more invasive and complex, therefore, as a rule, they begin with a man. The analysis of seminal fluid shows the general picture of the state of the male reproductive system. Often the result is a bad spermogram - what to do in this case?

Reliability of the result

A bad spermogram after the first donation does not always reflect the real state of affairs. Drinking alcohol for 3-4 days, strong physical exertion, caffeine, stress, toxic effects, overheating - all this can negatively affect performance. Spermatozoa in the vast majority will be inactive, defective or in too small a quantity.

To get the most reliable result, it is necessary to exclude all factors that provoke negative changes in sperm, and pass the spermogram again in a month or two. In the case of a serious illness, high temperature, the analysis is given after 3-6 months. If the spermogram is bad again, then you should consult a doctor (andrologist, reproductologist). There are family planning centers in almost every city. It is better to donate sperm where the assessment of seminal fluid is performed according to current WHO recommendations, and the morphology of spermatozoa is examined according to the Kruger criteria (staining of the preparation and 60x optical magnification).

Only a specialized laboratory for evaluating spermatogenesis with high-precision equipment, in which professional embryologists and spermiologists work, can conduct a reliable morphological diagnosis of spermatozoa. "General" laboratories, where all types of analyzes are performed, generally do not have such resources, their research, especially sperm morphology, gives very approximate results.

Spermogram is informative under three conditions:

  1. Diagnostics is carried out correctly, clearly according to the protocol, using high-quality equipment.
  2. The indicators are studied in dynamics.
  3. In parallel, other parameters of the functioning of the male body are evaluated.

Only according to the results of the spermogram obtained taking into account the above requirements, doctors can reasonably prescribe additional examinations and treatment.

Types of deviations in the spermogram, methods of treatment

What to do with a bad spermogram depends on the type of pathology and its severity. In some cases, to improve the results, it is enough for a man to refrain from alcohol and smoking for several months, avoid the toxic effects of external factors, take vitamins (A, D, E (Enat), C, zinc), balance nutrition, introduce seafood, nuts, mummy into the diet , dried apricots, increase physical activity, do not lie in a hot bath and do not visit the sauna. Often a positive effect is provided by drugs such as:

  • Speman;
  • Sperm Plant;
  • Carnicetin;
  • Spermstrong.

After some time, sperm motility will improve, their quality and quantity will increase.

But sometimes the causes of poor spermogram can be diseases that require diagnosis and medical or surgical treatment. In order not to waste time and financial resources on the purchase and intake of dietary supplements, it is necessary to immediately undergo several types of examinations in order to exclude serious pathologies. The standard list includes a blood test for sex hormones and ASAT, ultrasound of the scrotum, TRUS, palpation.

It should be borne in mind that pregnancy can occur even with low male fertility rates. Patients often tell doctors and write on forums: “My husband has a bad spermogram, but I got pregnant!”. There is nothing surprising. Sperm parameters are very variable, therefore, in one portion of the seminal fluid, deviations may be present, and in the other, after some time, the parameters are already sufficient for conception. Spermogram norms are designed for the IVF protocol; small shifts should not be taken as a serious pathology. Natural pregnancy is impossible only at critical values.

Few or no sperm

If there are less than 12 million spermatozoa in 1 ml of seminal fluid, this is considered a deviation from the norm. In this case, you need to pay attention to the level of sex hormones: if testosterone is produced little, then the production of germ cells slows down. Treatment consists in solving problems with the endocrine system, the use of hormone replacement therapy.

The reasons for the absence of spermatozoa or their small number in the seminal fluid may lie in the cicatricial transformations of the testicular tissues, which prevent the germ cells from leaving the organ. There is a risk of scarring if a man has had a disease such as chlamydia. Inflammation of the epididymis also negatively affects the patency of the vas deferens and sperm production. In some cases, medicines (Longidaza injections) contribute to the resorption of adhesions, microsurgery can help.

Absence of motile or live spermatozoa

If the spermatozoa do not move at all (akinospermia), then pregnancy will not occur, since they have no chance of getting to the egg and infiltrating it. The causes of the problem may be as follows:

  • internal intoxication with drugs, nicotine, alcohol, chemicals;
  • pathological processes of an autoimmune nature;
  • violation at the level of chromosomes;
  • inflammation;
  • diseases of the endocrine system.

The easiest way to improve spermogram parameters in the first case is to give up bad habits, protect yourself from exposure to toxins at work, and take supplements that stimulate spermatogenesis. Sperm motility will gradually recover.

If the spermatozoa do not move at all (akinospermia), then pregnancy will not occur.

If a karyotyping test showed chromosomal abnormalities, then, for example, if Klinefelter's syndrome (chromosome set 47 XXY) is detected, it will not be possible to cure a man. But with incomplete cell damage (mosaic), it is possible to extract live spermatozoa from the testicle using the micro-TESE method and perform artificial insemination.

Exceeding the norms of leukocytes and erythrocytes

The presence of leukocytes in the semen in excess of the norm indicates an active inflammatory process in the organs of the genitourinary system. It can be prostatitis, orchitis, epididymitis, tumors. Red blood cells can also be a sign of inflammation. Blood cells are also released due to the presence of stones in the prostate ducts.

According to the results of the examination, medical treatment is carried out, aimed at eliminating the source of pathology.

Many spermatozoa with pathological morphology

It should be noted right away that a “sick” sperm cell will not fertilize an egg, so you should not be afraid of conceiving a obviously sick child for this reason. If a husband has a bad spermogram, and a woman becomes pregnant, this does not mean that the baby will be born defective. In order to identify the pathology of an outwardly normal sperm cell, a deep study is required for chromatin fragmentation (“genetic material packaged” in the sperm cell). Scientific data on the relationship of poor morphology and congenital malformations of the fetus are lacking.

The rate of healthy sperm according to Kruger is 4%, if the result is significantly lower, then the probability of a natural pregnancy will be almost zero.

In the case when the structure of spermatozoa is reversibly disturbed, morphological indicators often improve after 2-3 months with the exclusion of toxic effects, including those due to the characteristics of the profession (solvents, gases, smoke). The regular intake of vitamins, el-carnitine has a beneficial effect on the parameters of the spermogram.

Fertilization by ICSI - conducting a spermatozoon into a pre-extracted mature egg

Artificial insemination

Treatment, both medical and surgical, does not always lead to clinically significant results. There is still a bad spermogram, showing negligible chances of conceiving naturally. But often, even with normal sperm, pregnancy stubbornly does not occur, then it is advisable for couples to resort to one of the types of artificial insemination.

The most harmless way for a woman's body, how to get pregnant without the direct participation of a man, is insemination. This requires the presence of a normative percentage of live active spermatozoa without morphological abnormalities, which are washed and injected into the uterine cavity on a day of the cycle favorable for conception.

If the spermogram is so bad that active spermatozoa are completely absent, then the couple has a way out in the form of fertilization by ICSI - leading the spermatozoon into the previously extracted mature egg. This is one of the IVF methods used in case of complete inability of the sperm to independently overcome the barriers (cell layers and membrane) to penetrate the egg. In classical IVF, the introduction of a sperm into an egg in the laboratory occurs without the intervention of specialists: in one test tube there are several full-fledged viable female and male germ cells at once.

Indications for ICSI:

  • pathologies of spermatozoa, making their penetration through the barriers of the egg impossible;
  • thickened egg shells;
  • immunological infertility (MAP test values exceed 50%);
  • failures after 2 or more classic IVF procedures.

ICSI is performed using high-precision equipment by embryologists. A full-fledged mature egg and the most suitable sperm for fertilization are selected, which is injected with a microneedle directly into the cytoplasm of the female cell.

ICSI does not exclude congenital pathologies of the unborn child. If the husband has a bad spermogram, the couple has hereditary diseases in the family, then it is recommended to undergo a medical genetic examination before the procedure. Significantly reduce the risk of pre-implantation diagnostics of embryos before replanting.

There are cases when a man is not capable of fertilization either by ICSI or by other methods (all spermatozoa are dead, defective or completely absent), then donor sperm can help.

A bad spermogram is a relative concept in cases where there is at least a small percentage of live spermatozoa. This diagnostic method is not informative and does not determine the possibility of conception. Even if the spermogram is unsatisfactory, additional tests should be done, for example: MAR test, EMIS, sperm biochemistry, DNA fragmentation.
