Pharmacological properties

Pharmacodynamics

Caffetin is a combined drug, the action of which is due to its components.

Paracetamol has an antipyretic and analgesic effect.

Caffeine stimulates the psychomotor centers of the brain, has an analeptic effect, reduces drowsiness, reduces fatigue, enhances the effect of analgesics, increases performance (both physical and mental), increases blood pressure and heart rate. Caffeine intake contributes to a more complete absorption of other active components of Caffetin.

Propyphenazone is characterized by analgesic and antipyretic action. Codeine has a central antitussive effect (suppresses the excitability of the cough center). The analgesic effect of the substance is a consequence of the excitation of opiate receptors in different parts of the central nervous system and peripheral tissues, which leads to stimulation of the antinociceptive system and a change in the emotional perception of pain.

The analgesic effect of the drug appears within 0.5-1 hour and lasts from 4 to 8 hours.

Pharmacokinetics

Paracetamol

Absorption of paracetamol is high. When taken orally, the maximum plasma concentration is observed after 0.5–2 hours. Approximately 15% of paracetamol binds to plasma proteins. This substance is able to penetrate the blood-brain barrier. Paracetamol is characterized by metabolism in the liver by conjugation with sulfates, glucuronides or oxidation by microsomal enzymes. In the latter case, the process proceeds with the formation of toxic intermediate metabolites with their subsequent conjugation with glutathione. Glutathione deficiency can lead to damage and necrosis of hepatocytes through exposure to toxic intermediate metabolites. The half-life is from 1 to 4 hours. Paracetamol is excreted by the kidneys in the form of metabolites, mainly conjugates.

Caffeine

Absorption when taken orally is good. Caffeine is absorbed throughout the intestines. After oral administration, the maximum plasma concentration is observed after 50-75 minutes. The substance is rapidly distributed in tissues and organs, easily penetrates the placental and blood-brain barriers. Approximately 25–36% binds to blood proteins (albumins). It is predominantly metabolized in the liver (over 90%). The half-life is 3.9-5.3 hours. The excretion of caffeine and metabolites is carried out by the kidneys.

Propyphenazone

Rapidly absorbed in the gastrointestinal tract. When taken orally, the maximum concentration in blood plasma is observed after 0.5-0.6 hours. It is metabolized in the liver, excreted as metabolites in the bile and urine. The half-life is from 2.1 to 2.4 hours.

Codeine

When taken orally, it is rapidly absorbed. The maximum plasma concentration occurs after 2-4 hours. Approximately 30% binds to plasma proteins. Metabolized in the liver to form active metabolites. Codeine is excreted in bile and urine. The half-life is from 2.5 to 4 hours.

Indications for use

According to the instructions, Caffetin is indicated for the relief of moderate pain of various etiologies:

  • Algodysmenorrhea;
  • Toothache;
  • Neuralgia;
  • Arthralgia;
  • myalgia;
  • Migraine;

  • Hematopoietic disorders;
  • Leukopenia;
  • Liver failure;
  • Deficiency of glucose-6-phosphate dehydrogenase;
  • kidney failure;
  • Increased excitability;
  • Insomnia;

The treatment period is up to 5 days.

Side effects

The use of Caffetin can cause side effects:

  • From the digestive system: nausea, increased activity of liver enzymes, gastralgia;
  • From the side of the nervous system: a decrease in the speed of psychomotor reactions, increased excitability (especially in children);
  • From the hemopoietic system: agranulocytosis, leukopenia;
  • Allergic reactions: itching, skin rash, angioedema, urticaria.

Overdose

In case of overdose, specific symptoms characteristic of each active component of the drug may occur.

Paracetamol

During the first 24 hours after ingestion, nausea, vomiting, pallor of the skin, abdominal pain, anorexia, impaired glucose metabolism and metabolic acidosis may occur. 12-48 hours after an overdose, signs of impaired liver function may appear. In adults, the hepatotoxic effect is manifested in the case of taking 10,000 mg of paracetamol or more.

Caffeine

When taking a daily dose of caffeine exceeding 300 mg (including against the background of consumption of more than 4 cups of natural coffee with a volume of 150 ml), anxiety, headache, tremor, confusion, extrasystole may appear.

Codeine

Symptoms of chronic and acute overdose: confusion, cold clammy sweat, severe weakness, dizziness, anxiety, nervousness, fatigue, drowsiness, hypothermia, slow shortness of breath, lowering blood pressure, bradycardia, convulsions, miosis.

Treatment

Donators of SH-groups and methionine (a precursor in the synthesis of glutathione) are recommended to be administered within 8-9 hours after an overdose, acetylcysteine - within 8 hours.

If you suspect poisoning, you should immediately consult a doctor.

special instructions

During treatment, you can not drink alcohol, as it can cause gastrointestinal bleeding.

During therapy, excessive consumption of drinks containing caffeine (tea, coffee) can cause nausea, tachycardia, tinnitus and other symptoms of an overdose of the drug.

For impaired liver function

It is forbidden to use Caffetin in the treatment of patients with liver failure.

Use in the elderly

In the treatment of elderly patients, Caffetin should be used with caution.

drug interaction

When Caffetin interacts with ethanol, antiepileptic drugs, zidovudine, barbiturates and rifampicin, the likelihood of hepatotoxicity increases, so it is not recommended to use the drug in these combinations.

With the simultaneous use of other drugs, the combined composition of Caffetin should be taken into account.

Caffeine, one of the active components of the tablets, accelerates the absorption of ergotamine, codeine enhances the effect of analgesics, hypnotics and sedatives.

Paracetamol - it is possible to slow down the excretion of chloramphenicol by 5 times, increase the activity of indirect anticoagulants (dicoumarin derivatives) in case of repeated administration of the drug.

The absorption of paracetamol is accelerated when combined with metoclopramide.

2.6. Organization of swimming training for preschool children

Swimming lessons for preschool children are organized in pools of various types, which are determined by their design features, conditions and operating features.

In practice, for teaching preschoolers to swim, it is advisable to use the following types of pools:

1. Covered small<*>pools:

In the buildings of preschool institutions or extensions to them (for one kindergarten, for several kindergartens);

In the buildings of cultural and sports complexes, swimming pools, departments and sports societies (the so-called "paddling pools");

In the buildings of children's sports complexes serving preschool institutions and primary classes of general education schools in the microdistrict;

In the buildings of children's clinics - for young children.

<*>Small-sized pools include all pools, the dimensions of which are less than those accepted for pools of sports complexes.

2. Open small pools in the areas of preschool institutions.

3. Indoor and outdoor sports-type swimming pools (track length - 25 and 50 m).

4. Collapsible pools.

5. On natural reservoirs - fencing of coastal areas of the bottom for classes; the simplest water stations with an artificial bottom (floating pools); outdoor pools on the shore of the reservoir. For teaching children to swim, it is advisable to have a pool with a size not less than that established by Building Codes and Rules<*>- 3 x 7 m, up to 80 cm deep. Such pools are suitable for primary swimming training for younger preschoolers. The experience of kindergartens shows that for organizing classes with a group of children of older preschool age, it is advisable to have large pools with a bathtub (12.5 x 6 m, depth - 1.2 - 1.5 m).

<*>SNiP II-64-80. Building regulations. Part II. Design standards. Chapter 64 Gosstroy of the USSR. M., 1981.

The most convenient form of the pool - rectangular. Depending on local conditions, other forms of pools (square, oval, round) are also possible. To lower children into the water, it is necessary to provide for a vertical ladder with handrails. It is advisable to cover the steps of the stairs with ribbed rubber or other non-slip material.

The bathtub is finished with plain facing tiles. At the pool there should be at least two dressing rooms equipped with hangers for clothes and towels, banquettes, sofas. Rubber mats or wooden gratings are placed on the floor in the dressing rooms.

At the pool, shower rooms are required, with at least 2 - 4 shower units for washing the body of children before entering the pool and at the exit from it, and toilets. The number of showers, changing rooms, toilets is determined by the capacity of the pool. Facilities for drying children's hair should also be provided in the pool area.

In order to ensure the safety of children and prevent injuries, rubber mats are placed when leaving the pool and from the shower so that children do not slip on the wet floor.

All premises allocated for preschool children must meet sanitary and hygienic requirements, be equipped with appropriate furniture, hangers for linen, towels, bathing and toiletries. When equipping the premises, measures must be taken to ensure the prevention of child injuries.

In any case, the dimensions of the pool should allow classes with a group of children of at least 8 - 10 people.

Swimming lessons for children in outdoor pools are organized during the summer months. Depending on climatic conditions, the bathing time may be different. In the middle lane - 2 - 3 months, in the southern regions - up to 3 - 4 months.

When pools on the plots are necessary: shower installations; foot baths before entering the pool (0.8 - 1 x 0.1 m), places for undressing and dressing children, which must be equipped with benches and sofas with hygienic coating, towel racks.

The most appropriate location of the pool on the site should be considered its equipment in a place where the entire surface of the water is heated by the sun. This is especially important if the pool water is not heated.

Pools of prefabricated structures can be installed outdoors, their operating conditions should be close to those of open small-sized pools.

In the event that there is no pool in the kindergarten, you need to prepare a place for swimming and teaching children to swim in the river, lake, sea. When choosing a bathing place, one should take into account the nature of the reservoir and the shore, the condition of the bottom, the speed of the water flow, as well as the distance from the reservoir to the kindergarten. It must be coordinated with the city or district OSVOD and the sanitary service.

The section of the reservoir for bathing children should have a flat sandy bottom, be dense and non-silty.

The bottom of the reservoir should be examined in advance and cleaned of foreign objects - stones, snags, algae and other objects. There should be no keys in the bathing area for children.

The transition from a smaller depth to a larger one should be uniform, gentle, with a gradually lowering bottom.

For children of younger preschool age, the depth of the reservoir should not exceed 60 cm, for older children - 80 - 90 cm, the flow speed - no more than 0.5 m / s.

The water in the reservoir should be clean and transparent. The suitability of water for bathing children is established by the sanitary supervision authorities. You can conduct classes with children at an air temperature of + 24 - 25 degrees. C, water - +23 degrees. C.

For classes, children are divided into subgroups of 12 - 15 children. The duration of the lesson, together with preparation for children of different preschool ages, is recommended from 10 - 15 to 30 - 40 minutes per subgroup (no more than 3 - 5 minutes on land).

Hygienic requirements for the pool premises, their equipment and water quality

Swimming lessons with preschool children should be held in pools that meet all sanitary and hygienic requirements. They provide: effective ventilation, sufficient illumination of all rooms; a favorable level of temperature and air, cleanliness and order are maintained.

In accordance with the Sanitary Rules<*>the air temperature in the pool room is maintained at +29 degrees. C; in the dressing room and shower room - within +25, +26 degrees. C, water in the pool - +27, +30 degrees. C. The floor in the children's pool is heated, the passages are heated, and measures are taken to prevent drafts.

<*>Sanitary rules for the arrangement and maintenance of places of employment in physical education and sports. No. 156776. M., 1980.

The water supplied to the pool must, in all respects, comply with GOST 2874-82 "Drinking Water" and meet the following requirements: turbidity - no more than 1.5 mg / liter; color - no more than 20 degrees; transparency - not less than the depth of the bath (on the cross); smell and taste - no more than two points; residual chlorine - 0.3 - 0.5 mg / l; if titer - no more than 3; ammonia content is allowed no more than 0.5 mg/l; oxidizability - 4 mg/l.

Disinfection of water in the pool, regardless of its type, size, water filling system is required.

In small-sized children's pools, draining and disinfection, subject to a constant influx of fresh water of at least 10 - 15% of its volume, should be carried out at least 1 time in 10 days.

With a daily change of water, the method of its disinfection, the regularity of quality analyzes, the presence of residual chlorine, as well as the system for cleaning the pool bowl are established by the district sanitary and epidemiological station, taking into account the number of groups involved (throughput), the size of the pool.

Bath disinfection is carried out with each emptying of the pool. It is thoroughly washed with a soap and soda solution, brushed, followed by rinsing with hot water from a hose.

In accordance with the existing recommendations (Collection of basic instructive and methodological materials on the hygiene of children and adolescents. USSR Ministry of Health. Main Sanitary and Epidemiological Department. M., 1975), daily cleaning is carried out in the pool premises, airing is carried out, and current disinfection of the premises is carried out. Disinfection is subject to: bypass paths, floors, benches in locker rooms, foot bars or rubber mats, door handles, handrails. They are wiped with a rag moistened with a 0.5 - 1.1% solution of chloramine. Panels are periodically disinfected to a level of at least 2 meters from the floor in the pool hall, utility rooms (during sanitary days) as the final stage of general cleaning.

Current control over the sanitary condition of the main and auxiliary premises of the swimming pool, regular decontamination is carried out by medical workers of the sanitary and epidemiological station.

In the event that the swimming area for children is part of a sports pool, it must have its own separate cleaning and disinfection system.

The requirements for water quality and sanitary condition of an outdoor pool should be similar to those for indoor pools.

Inventory and equipment

Indoor and outdoor swimming pools must be equipped with devices that allow monitoring and maintaining an appropriate sanitary and hygienic condition, thermometers for measuring air and water temperature, and psychrometers for determining air humidity.

At the pool, it is necessary to have equipment and equipment to ensure the safety of swimming and insurance of children during classes:

A long pole (2 - 2.5 m) to support and insure swimmers;

Lifebuoys (1 - 2);

Float "baby";

Dividing paths for fencing the bathing area;

Colored floats, flags (to protect the bathing place on the river, the sea and the location of children on land).

There is also a need for other aids and toys designed for the most effective organization of swimming lessons, conducting various exercises and games that contribute to the successful development of children in the water and the formation of swimming skills.

These include:

a) foam boards (for each student);

b) inflatable toys;

c) small toys made of dense rubber (sinking);

d) hoops.

All aids and toys must be sanitized in accordance with sanitary and hygienic requirements.

Requirements for children's clothing, bathing and toiletries

When visiting the pool, each child must have the following bathing accessories: swimming trunks, rubber cap, swimming slippers, towel, rubber sponge, washcloth, soap, comb.

When preparing for a swimming lesson, children should undress in their dressing room to their underwear, put on their flannelette robe or cape over it, socks and slippers on their feet; take your swimwear and go to the pool.

Before the start of classes, children are invited to visit the toilet. Then they take off their clothes, fold or hang them as quickly and neatly as possible. Children should shower thoroughly with soap and a washcloth (sponge) before entering the water. The water temperature of the hygienic shower is +36 degrees. C. After that, the children take a cold shower for a while (about 30 degrees C). At the exit points from the shower to the bypass path, a walk-through foot bath is provided.

Leaving the pool, children take a warm shower, the temperature of which is 2 - 4 degrees higher than the temperature of the water in the pool. After swimming and showering, they carefully dry themselves with a towel, dry their hair.

Example 1. General purpose group. The number of children in the group over the age of three years - 20 people.

The working hours of the preschool institution are 6 days a week, with 9-hour stay of children, 10% - planned absenteeism according to the accounting department.

9x6x1.1 = 59,4 = 1,65.
36 36
9x6x1.1 = 59,4 = 1,49.
40 40

Example 2. General purpose group. The number of children under the age of one year - 10 people, 10% - planned absenteeism.

The working hours of the preschool institution are 5-day working week with 24-hour stay of children.

Number of teachers in the group:

2.6. Organization of swimming training for preschool children

Swimming lessons for preschool children are organized in pools of various types, which are determined by their design features, conditions and operating features.

In practice, for teaching preschoolers to swim, it is advisable to use the following types of pools:

1. Covered small<*>pools:

In the buildings of preschool institutions or extensions to them (for one kindergarten, for several kindergartens);

In the buildings of cultural and sports complexes, swimming pools, departments and sports societies (the so-called "paddling pools");

In the buildings of children's sports complexes serving preschool institutions and primary classes of general education schools in the microdistrict;

A long pole (2 - 2.5 m) to support and insure swimmers;

Lifebuoys (1 - 2);

Float "baby";

Dividing paths for fencing the bathing area;

Colored floats, flags (to protect the bathing place on the river, the sea and the location of children on land).

There is also a need for other aids and toys designed for the most effective organization of swimming lessons, conducting various exercises and games that contribute to the successful development of children in the water and the formation of swimming skills.

These include:

a) foam boards (for each student);

b) inflatable toys;

c) small toys made of dense rubber (sinking);

The standard working time of the teacher's assistant per week in groups:

For children with tuberculosis intoxication - 36 hours;

For mentally retarded children and children with damage to the central nervous system and mental disorders - 36 hours;

In other cases - 40 hours.

The attendance of educators and assistant educators in the group is calculated by the formula:

chiav =

Ts x T

Tn

Where:

Chyav - the number of educators or assistant educators in the group;

Тс - duration of stay of children in the group per shift;

T - the number of working days of the children's institution per week;

Tn - the standard duration of the working time of the educator and assistant educator per week.

The payroll number of employees (NSP) is determined by the formula:

where % of planned absenteeism is determined according to accounting data.

Example 1. General purpose group. The number of children in the group over the age of three years - 20 people.

The working hours of the preschool institution are 6 days a week, with 9-hour stay of children, 10% - planned absenteeism according to the accounting department.

Swimming lessons for preschool children are organized in pools of various types, which are determined by their design features, conditions and operating features.

In practice, for teaching preschoolers to swim, it is advisable to use the following types of pools:

1. Indoor small pools:

In the buildings of preschool institutions or extensions to them (for one kindergarten, for several kindergartens);

In the buildings of cultural and sports complexes, swimming pools, departments and sports societies (the so-called "paddling pools");

In the buildings of children's sports complexes serving preschool institutions and primary classes of general education schools in the microdistrict;

In the buildings of children's clinics - for young children.

Small-sized pools include all pools, the dimensions of which are less than those accepted for pools of sports complexes.

For teaching children to swim, it is advisable to have a pool no smaller than the size established by the Building Code - 3 x 7 m, up to 80 cm deep. Such pools are convenient for primary swimming training for younger preschoolers. The experience of kindergartens shows that for organizing classes with a group of children of older preschool age, it is advisable to have large pools with a bathtub (12.5 x 6 m, depth - 1.2 - 1.5 m).

SNiP II-64-80.

Swimming lessons with preschool children should be held in pools that meet all sanitary and hygienic requirements. They provide: effective ventilation, sufficient illumination of all rooms; a favorable level of temperature and air, cleanliness and order are maintained.

In accordance with the Sanitary Rules, the air temperature in the pool is maintained at +29 degrees. C; in the dressing room and shower room - within +25, +26 degrees. C, water in the pool - +27, +30 degrees. C. The floor in the children's pool is heated, the passages are heated, and measures are taken to prevent drafts.

Sanitary rules for the arrangement and maintenance of places of employment in physical education and sports. No. 156776.

A long pole (2 - 2.5 m) to support and insure swimmers;

Lifebuoys (1 - 2);

Float "baby";

Dividing paths for fencing the bathing area;

Colored floats, flags (to protect the bathing place on the river, the sea and the location of children on land).

There is also a need for other aids and toys designed for the most effective organization of swimming lessons, conducting various exercises and games that contribute to the successful development of children in the water and the formation of swimming skills.

These include:

a) foam boards (for each student);

b) inflatable toys;

c) small toys made of dense rubber (sinking);

d) hoops.

All aids and toys must be sanitized in accordance with sanitary and hygienic requirements.

Answer

Answer to the question:

“Standards for determining the number of personnel engaged in servicing preschool institutions (nurseries, nurseries, kindergartens, kindergartens)”, approved by the Decree of the Ministry of Labor of the Russian Federation of April 21, 1993 N 88, have not been canceled and continue to operate in a general manner.

The above decision, as well as the standards themselves, have not been canceled. In addition, it should be noted that the letters of the Ministry of Labor are not regulatory legal acts, that is, documents that are binding and legally binding. Such letters are issued to clarify controversial issues of legislation and are advisory in nature. This follows from Article 5 of the Labor Code of the Russian Federation, Regulations on the Ministry of Labor and Social Protection of the Russian Federation, approved by Decree of the Government of the Russian Federation of June 19, 2012 N 610.

Thus, the “Standards for determining the number of personnel employed in servicing preschool institutions (nursery, nursery-kindergartens, kindergartens)”, approved by the Decree of the Ministry of Labor of the Russian Federation of April 21, 1993 N 88, can be applied in a general manner.

Details in the materials of the System:

Situation: How to determine the number of employees of an organization or department.

The number of employees of an organization (separate subdivision) is determined by its head in accordance with the structure of the organization, its functions and levels of management.

As a general rule, employers are free to determine both the staffing by positions and professions, and the number of employees performing a particular labor function.

At the same time, for certain types of activities, as well as institutions and organizations, there are statutory headcount standards that are mandatory for execution. To a greater extent, this applies to government agencies and departments. So, for example, the structure and staffing of the central office of the federal executive body is approved by its head within the limits of the wage fund and the number established by the president or the Government of the Russian Federation, taking into account the register of positions and other acts that determine the standard number of the relevant units (). Another example is the procedure for forming the structure and staff of the Central Office of Rospotrebnadzor ().

At the same time, there are a number of legislative acts to determine the staffing level, which are advisory in nature. In particular, these are provided for budgetary organizations. In this document, the calculation of the staffing is based on labor standards, on the basis of which the required number of employees is calculated to perform a particular labor function.

The number of employees based on labor standards can be determined by the formula:

The normative fund of working time of one employee per year is conditionally taken as 2000 hours.

An example of calculating the number of employees of a structural unit

The organization provides laundry services and opens a new structural unit - a laundry. To calculate the staffing level, personnel department employees are guided by approved ones.

The planned indicators for the operation of the new laundry are defined as follows:

  • The amount of processed linen per shift - 12,000 kg;
  • Laundry operating hours - 1-shift.

The optimal number of employees for the new laundry was determined by the HR specialist as follows:

The number of laundry employees, according to the Standards for the number approved, is 1.7, that is, 2 people.
